The Care Management Coordinator, Documentation Improvement actualizes the vision, mission, values, balanced scorecard performance measures for Montefiore Medical Center. She/he maintains standards of the practice for nursing as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice and Nursing’s Social Policy Statement.The Care Management Coordinator, Documentation Improvement [CMC-DI] is a Registered Professional Nurse with a sound base of current clinical expertise/knowledge and understanding of coding principles and guidelines. She/he reviews medical records to identify documentation improvement opportunities and then interacts with providers aiming for the provider to document the clarification within the body of the record. The CMC-DI is responsible for improving the overall quality and completeness of clinical documentation so it best reflects a patient’s severity of illness and risk of mortality and will therefore support appropriate reimbursement. Extensive interaction with providers at all levels is a large part of this position. Additionally, the CMC-D/I participates in performance improvement (PI) projects designed to continually improve documentation efforts at the medical center.

 

Education/Skills:

 

This associate is a Registered Nurse with current NYS Registration. S/he has, at minimum, a Bachelor of Science in Nursing or a Bachelor’s degree in a related field.
She also has a minimum of 5 years recent [defined as within the past 8 years] acute care clinical experience [excluding Psychiatric, Acute Rehab, Obstetrics or Newborn].
In addition s/he will have a minimum of 2 years critical care experience preferred.
This RN conducts Documentation Improvement activities by reviewing medical records on a concurrent basis to assess the extent to which clinical documentation accurately reflects the severity of patient illness and the scope and intensity of related
services/procedures. S/he is required to demonstrate excellent clinical knowledge of disease processes, including, but not limited to, symptomatology, diagnostics and treatment. S/he is required to demonstrate knowledge of the difference in symptoms
vs clinical diagnosis. S/he is required to demonstrate knowledge of federal, state and commercial/third party regulations related to coding and reimbursement. S/he works collaboratively with physicians and HIM associates to ensure compliance with coding
and reimbursement guidelines for inpatient admissions. The RN queries physicians or physician extenders to clarify diagnosis and procedures to support accurate Diagnosis Related Group assignment. This associate also demonstrates excellent
interpersonal and communication skills and displays a commitment to interdisciplinary teamwork. S/he is required to demonstrate proficient knowledge in software systems, including, but not limited to, Microsoft suite.

 

 Days vary.  Work week with Occasional Weekends Required, as permitted by the collective bargaining agreement
