Assesses and provides direct care to a designated group of patients. Develops, delegates and evaluates the care plans for patients in collaboration with other disciplines. Orders, performs and interprets diagnostic procedures and tests. Prescribes and administers medications.

 

Family Nurse Practitioner Fellow:

The Nurse Practitioner Postgraduate Fellowship in Primary Care provides a highly structured transition from academic preparation to practice that supports the development of confidence, competence, and mastery in the primary care in the FQHC setting. This 12-month, benefitted, full-time, position is a unique employment and training opportunity for NPs to start their practice career in a high-performance health system and in a supportive learning environment. Upon graduation from this program, the NP fellow will be able to work independently, providing evidenced based care to the medically under-served.

The fellowship will include precepted and mentored primary care sessions with experienced nurse practitioners and physicians, weekly didactic sessions, as well as limited specialty rotations.

 

The mission of the Montefiore Medical Group's Advanced Practitioner Fellowship is to expand access to primary care by equipping new clinicians with the skills needed to provide high quality, team-based, equity driven primary care in the service setting of medically and socially complex urban community clinics in the Bronx leading to the overall advancement of health and promotion of positive health outcomes in the communities served by MMG.

Responsibilities:

  • Develop and manage a panel of patients with the exclusive attention of an expert primary care provider
  • Perform a comprehensive, evidence-based assessment using advanced clinical judgment to diagnose
  • Works as part of an integrated primary care team focusing on diversity of chief complaints, efficiency, episodic and acute care
  • Demonstrate effective interpersonal and communication skills
  • Use a variety of learning experiences that may include, but are not limited to, didactic sessions, case presentations, clinical rounds, and mentored clinical experiences to demonstrate the ability to provide care proficiently and confidently for individuals and populations with complex conditions and/or multiple needs
  • Participate in weekly didactic sessions focused on topics that are high complexity/risk/burden and commonly seen in primary care

 

 Requirements:

  • Completion of supplemental application questionnaire to be emailed applicant upon receipt of application
  • Master’s Degree
  • Licensed (NY APRN) and credentialled (FNP-C or FNP-BC) or license/credential eligible as FNP by program start date (September 2024 – August 2025)
  • Graduate (less than 48 months) from accredited Master of Science in Nursing(MSN) or Doctor of Nursing Practice(DNP) program with a specialty as a Family Nurse Practitioner
  • Must demonstrate ability to provide primary care for all age groups-women’s health, pediatrics, dermatology, orthopedics, HIV/Hep C care, healthcare for the homeless, adult behavioral health, child/adolescent behavioral health, newborn nursery, and geriatrics
  • Must have interest in gaining experience working in a community health center setting.  Fellows should be self-directed and enthusiastic learners who desire and are receptive to feedback to advance their practice as well as desire further learning and understanding in basic and advanced family practice care topics
  • The selected candidate is expected to make a commitment to stay at Montefiore for a period of 2 years post-completion of the program.

 

Shift: Monday – Friday 8:30a-5:00p

Location varies with local travel within the Bronx and Yonkers required.

Compensation: Maintain existing salary of any RN who is selected for the program. External hires shall receive $85,000 annually. 

 

Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek.
