Bronx Equity - Integrated Care for Kids is a CMMI grant awarded to NYSDOH and where Montefiore was selected to be the lead organization in the operationalization of the grant and implementation of the intervention of conducting population health management strategies to the pediatric population with the catchment area. BE-InCK NY's goal is to improve the quality of care for children covered by Medicaid in the Bronx through population health management and provider engagement.  The Project Analyst functions as lead on assigned projects and supports various writing deliverables,  educational efforts towards the partnership council and other program activities. Specifically, the Project Analyst leads the Grant Team in developing the data exchange strategy; leads the collaboration in matters pertaining to operational reports, data exchange and program measurement; leads internal data analysis and process measurement efforts; provides subject matter expertise and technical assistance to Service Integration Coordinators to ensure fidelity to approved standing operating procedures; co-leads partnership council engagement as required by the Notice of Funding and facilitates the compliance with the program requirement to develop an Alternative Payment Model (APM).

 

The BE-InCK NY project and Project Analyst position are grant-funded through 2026. Annual renewal of grant is required.

 

 Bachelor's Degree required, Master's Degree preferred

 2 Years experience

 Strong project management, communication, and organizational skills

 Ability to develop and implement health care integration program policies and procedures

 Ability to train and organize project champions in program implementation, quality improvement, data sharing, and maintenance of program fidelity

 Demonstrated ability to collaborate and communicate effectively in a team setting

 Ability to organize and analyze data from various EMRs, HIEs, and documentation platforms for operational reports to inform eligibility criteria and program progress

 Ability to maintain effective and professional relationships with members of the project team, partner agencies, and state colleagues

 Ability to produce and edit health education material for internal and external use

 Working knowledge of core child services areas

 Experience with presentations in small and large groups

 Experience working with the Bronx population
