SERVICE INTEGRATION SPECIALIST

YONKERS, NY, US


BE-InCK NY Service Integration Analysts (SIAs) will coordinate completion of the BE-InCK NY Needs Assessment process to identify the core child service needs (medical, behavioral, food, housing, educational, social, etc.) and service integration level of the BE-InCK NY attributed population. SIAs will serve as resource navigators and coordinate access to needed core child health or social services for attributed members and their families. For attributed members with higher needs, SIAs will be responsible for providing care coordination and serving a as single point of contact (SPOC) to support improved access to services, resources or programs to meet identified needs, establish a comprehensive Plan of Care and increase information sharing amongst the care team for all assigned members and their families. SIAs will be expected to comply with all BE-InCK NY grant, program and documentation requirements as dictated by oversight entities and grant administrators. 

The BE-InCK NY project and SIA positions are grant-funded through 2026. Annual renewal of grant is required. 

 

  • Associates Degree required, Bachelor’s degree preferred in a health/human services related field with 2 years of relevant experience providing care coordination to children, adolescents, and families in the community.
  • Exceptional communication and interpersonal skills.
  • Must possess excellent organizational, follow-up, and record keeping skills.
  • Ability to operate independently using telecommunications and computer access to a database.
  • Ability to engage and collaborate with medical, behavioral health, specialty, educational and social service providers to support member's functioning (address core service needs).
  • Working knowledge of community-based resources and public benefits.
  • Outside of traditional business hours (8:30-5 pm) as per enrollee's availability/needs.
  • Experience utilizing EHR, RHIO/HIE and other statewide databases preferred
